Project Homeless Connect

New Horizons Behavioral Health, through its PATH Program, assists individuals and families experiencing homelessness.  Each January PROJECT HOMELESS CONNECT brings together community resources in a One-Stop Shop to address the needs of the homeless. 

Stable housing provides the foundation upon which people build their lives. Without a safe, affordable place to live, it is almost impossible to achieve good health or to achieve one’s full potential.

People with mental and/or substance use disorders can be particularly vulnerable to becoming homeless or being precariously housed. According to HUD’s 2016 Annual Homelessness Assessment Report, one in five people experiencing homelessness had a serious mental illness, and a similar percentage had a chronic substance use disorder. 

New Horizons Behavioral Health provides effective treatment and evidence-based interventions for individuals and families experiencing homelessness.

The Point-in-Time (PIT) count is a count of sheltered and unsheltered homeless persons on a single night in January. HUD requires that Continuums of Care conduct an annual count of homeless persons who are sheltered in emergency shelter, transitional housing, and Safe Havens on a single night. Continuums of Care also must conduct a count of unsheltered homeless persons every other year (odd numbered years). Each count is planned, coordinated, and carried out locally. The Housing Inventory Count (HIC) is a point-in-time inventory of provider programs within a Continuum of Care that provide beds and units dedicated to serve persons who are homeless, categorized by five Program Types: Emergency Shelter; Transitional Housing; Rapid Re-housing; Safe Haven; and Permanent Supportive Housing.

In  2018 the United Way of the Chattahoochee Valley’s Home For Good program conducted over 300 surveys for the annual Point In Time (PIT) Count. The survey sheds light on the situations of local sheltered and unsheltered homeless individuals, and gathers information to help those in need as well as develop more effective homelessness prevention programs.
